Filing a Formal Complaint
If you have made an appeal to the OIS Project Manager or to the OIS Steering Committee regarding:
- Resolution about a Complaint or Issue with a Certified Instructor, or
- Resolution about Decertification of OIS Instructor, or
- Maintaining OIS Instructor Certification, or
- Denial of acceptance to attend or become an Instructor Training, and
you still believe that you want to file a formal complaint against the decision made by the OIS Steering Committee or OIS Project Manager, you may complete the OIS Instructor/Candidate Complaint Form. The complaint form must be submitted to the current contractor for the Oregon Intervention System Executive Director.
If resolution is not reached within thirty (30) working days, the OIS Contractor’s Executive Director will submit the complaint to the ODDS.
